Communication-confusion, disorientation, forgetfulness, anger or hostility

Economic Condition-confusion over money matters

Social Condition-isolated from social contact

Emotional Health-extreme depression over the loss of a spouse, friend, relative or pet. experiencing withdrawal, rejection or anxiety

Personal Appearance-unkempt or dirty

Physical limitations-severe difficulty in seeing, hearing, speaking or moving about

Condition of the home-dire need of repairs, offensive odors, unattended pets
